      • Post-surgery infections
      • Unnecessary operations, surgery and/or procedure
      • Wrong site wrong body part or organ, left or right confused
      • Wrong patient

    II. Errors of Omission which include the following:

    • Failed to treat a condition or disease
    • Critical drug not given
    • Critical procedure not performed
    • Inadequate screening
    • Wrong blood type, possible bad transfusion
    • Inadequate donor blood testing
    • Blood contaminated by HIV, hepatitis, etc.
